Geriatric Care Managers (GCM) are uniquely trained and skilled as patient advocates and as a health care resource for the patient and their family.

The in-depth training GMC’s obtain is in nursing, social work, or gerontology.  They usually have years of professional experience and extensive knowledge about the costs of senior services, various types of insurances, and the quality and availability of care in the community.

GMC’s will assist with discharge transitions from hospital to home.

They will coordinate the multi-agency care that is required by adults with chronic illnesses or seniors who may have physical or mental impairments.  The goal of the GCM is to collaborate with the client and/or family to meet the long term care needs, improve quality of life, and maintain or restore a prior functional level of independence for each patient.

As a licensed private duty home care agency, AA Care Services employs an RN Geriatric Care Managerto perform the initial assessment for adults or seniors requiring assistance with their long term care needs.

Typically, the services that are required include activities of daily living (ADL) or instrumental activities of daily living (IADL).

  • ADL’s include bathing, grooming, and assistance with safe mobility or transfers
  • IADL’s are those tasks that help keep the senior safely at home and include meal preparation, light housekeeping, medication reminders, grocery shopping, assistance with paying bills, pet care, etc.
